With this request, you are authorizing MyCarrier, LLC to send invoices to the provided company name for all cargo insurance premiums and all fees associated with the purchase of those premiums from Ursus Insurance Services Inc. (DBA Loadsure) via our product, MyCarrierTMS. Failure to pay the invoices in accordance with the terms outlined on the invoice may result in the loss of access to credit terms for future cargo insurance purchases via MyCarrierTMS. Continued failure to pay after notice may result in suspended access to MyCarrierTMS.
